Fentanyl is a highly potent synthetic opioid. Illicitly manufactured fentanyl is commonly present in heroin, counterfeit pills, and sometimes stimulants, often without the person’s knowledge. This increases the risk of rapid, fatal overdose.
Fentanyl use disorder is treated as opioid use disorder. Effective treatment includes FDA-approved medications—buprenorphine, methadone, and naltrexone—along with individualized behavioral health and recovery support.
Buprenorphine and methadone reduce mortality and support retention in care. Because fentanyl may remain in the body’s tissues and can complicate traditional buprenorphine initiation, clinicians may use different evidence-informed induction strategies based on symptoms, setting, and experience.
A return to use should prompt rapid reassessment rather than automatic discharge. Ongoing medication, higher-intensity care, and harm-reduction planning may be needed.
Anyone at risk should have naloxone available and ensure people nearby know how to use it. More than one dose may be needed. Call 911, provide rescue breathing when trained, and remain with the person. Avoid using alone and be cautious with counterfeit pills, which may contain fentanyl regardless of appearance.
