Benzodiazepines are prescription medications used for conditions such as anxiety, insomnia, and seizures. Physical dependence can develop even when they are taken as prescribed. Dependence is not the same as a substance use disorder, although both can occur.
Abruptly stopping benzodiazepines after regular use can cause severe withdrawal, including seizures and delirium. A gradual, individualized taper under medical supervision is generally recommended when risks of continued use outweigh benefits.
Treatment begins with a review of the specific medication, dose, duration, prescribing history, co-use of alcohol or opioids, prior withdrawal, seizure risk, psychiatric symptoms, and medical conditions. Tapers may take weeks to months or longer. The pace should be adjusted to symptoms and safety rather than forced into a rigid schedule.
Some people require a higher level of care, particularly with severe withdrawal risk, unstable medical or psychiatric conditions, polysubstance use, or unsuccessful outpatient taper attempts.
Care may include treatment for anxiety, insomnia, trauma, or other conditions that led to benzodiazepine use. Non-benzodiazepine medications and behavioral therapies may be considered. Combining benzodiazepines with opioids, alcohol, or other sedatives raises overdose risk.
