Buprenorphine is an FDA-approved medication for opioid use disorder (MOUD). It is a partial opioid agonist that can reduce withdrawal symptoms and cravings and can lower overdose risk when taken as prescribed. Some formulations combine buprenorphine with naloxone; Suboxone is one brand name, but generic and other formulations are also available.
Buprenorphine treatment should be individualized. It may be started after objective withdrawal develops, or through alternative induction approaches selected by an experienced clinician. Because fentanyl can complicate timing, patients should follow a clinician-directed plan rather than attempting induction without guidance.
Care typically begins with an evaluation of opioid use, withdrawal history, current medications, pregnancy status, medical and psychiatric conditions, and treatment goals. Follow-up includes dose adjustment, side-effect review, overdose prevention, and coordination with counseling or other supports when desired and appropriate.
There is no single correct treatment duration. Continuing buprenorphine for as long as it provides benefit is appropriate, and discontinuation should be gradual and clinically supervised.
Buprenorphine can still cause sedation and respiratory depression, particularly when combined with alcohol, benzodiazepines, or other sedating drugs. Patients should tell clinicians about all substances and medications they use. Prescribers should discuss safe storage, diversion prevention, and naloxone.
Stopping medication reduces opioid tolerance and may increase overdose risk if opioid use resumes.
