Alcohol use disorder is a medical condition characterized by difficulty controlling alcohol use despite negative effects. Severity ranges from mild to severe. It is not defined by a particular type of drink, daily use, or a lack of willpower.
Effective treatment may include behavioral therapy, medications, mutual-help or peer support, case management, and treatment for co-occurring mental health and medical conditions. The recommended level of care depends on withdrawal risk, drinking pattern, prior treatment, health conditions, safety, and recovery environment.
Alcohol withdrawal can be life-threatening. Symptoms may include tremor, sweating, anxiety, nausea, elevated heart rate, hallucinations, seizures, or delirium. People with a history of severe withdrawal, seizures, heavy prolonged use, major medical illness, or pregnancy need prompt clinical assessment. Do not stop heavy alcohol use abruptly without medical guidance when withdrawal risk is possible.
Counseling approaches may include motivational interviewing, cognitive behavioral therapy, relapse-prevention treatment, and community reinforcement. FDA-approved medications include naltrexone, acamprosate, and disulfiram; other medications may be used off-label when clinically appropriate.
Treatment goals may include abstinence or other clinically meaningful reductions in alcohol-related harm, depending on safety, diagnosis, and informed patient preference.
